Debate on Transgender Care in Netherlands

The final part of tonight's news of the day, where we'll be talking to

our opinion writer. Tonight, that's Wierd Duk. And Wierd,

you want to talk about transgender care in the Netherlands.

This summer, the debate was stirred up again, partly by a group of

opinion writers in Trouw. Pieter Omtzigt was among them.

Yes, and a number of medical specialists too,

of course.

Yes, them too. Yes, absolutely. Armand Girbes, among others.

They said, yes, you can indeed be dissatisfied with your

sex and perhaps want a

different sex. But that feeling can also go away.

Well, that's what they say. That's what they say. But the letter is mainly intended

to draw attention to the fact that the Dutch Protocol,

as it's called, is a method, a methodology, so to speak,

that was developed years ago

developed for transgender children. How should we deal with that medically?

That was the standard, so to speak, for years.

It was the international standard for dealing with these children with

gender dysphoria, as it's called. But in many countries, they have

backtracked on that because, first, the medical evidence for that approach,

well, that evidence is very weak, and moreover, the

results of it, namely that the

gender dysphoria will go away. If, for example, you have yourself transitioned from

a boy to a girl, that evidence is weak, too.

That evidence is also very weak. And now, in

a considerable number of countries, the

Dutch Protocol has been withdrawn, or even in some countries, that method has been

banned.

For example, England, Denmark, Finland, and Sweden.

The Scandinavian countries, yes. Yes,

In the United States, as well as in many states and

the Netherlands is behind in that regard.

the Netherlands is behind in that regard.

said, well, actually, the Dutch

Protocol

still stands, right, it can remain in place, we can continue it like this

because we are very

careful and cautious with those children.

Yes, because they say that truly individualized care is provided, right.

So, yes, those hospitals don't take this lightly.

That's right. Whereas the authors say

come on, you have to look into it. You need to provide these children with much more

psychological

support. You need to support them socially. Where does that desire come from?

If you are fourteen, fifteen years old or sometimes even younger, and

you are a girl, and then

you think that you want

to become a boy.

become.

What role do social media play in this?

We’re talking about social contagion, which plays an important role here.

And I found that important because this kind of criticism is dismissed in the Netherlands,

and elsewhere in the world too, for now, but in the Netherlands very quickly

as transphobia, for example.

If you criticize transgender activism, you’re a transphobe.

And then people say you’re denying these people’s existence.

Yes, you are indeed denying their identity and

their existence.

While the consequences for those children, for those who ultimately undergo

transition.

That Dutch Protocol consists of the following. First, you get puberty blockers.

Puberty is postponed. The idea is that they can think about it for a little

longer.

I do want this, though. And then you receive a

hormone treatment.

Now, the reality is that during that pause button, so to speak,

of those puberty blockers, a very large—by far the

largest number of these children say: I still want that hormone treatment.

Then you receive hormonal, hormonal treatment, that is, with hormones.

Then comes the medical stage when you

are going to undergo surgery. So breasts are removed,

your sex is

removed. In my view, bizarre procedures

are performed to then turn

a boy, for example, into a girl.

And then imagine that such a child, when they are eighteen, nineteen,

twenty, or twenty-one, regrets it. What happens then.

Terrible.

Moreover, we ultimately don't know exactly what the consequences are of

those medical procedures on the bodies of those children.

Your point now would be, wait a minute.

Wait, for God's sake, don't do it.

So, don't give puberty blockers to minors. That hormone treatment.

Wait until they're eighteen, for example.

Well, for example. And ultimately, when you're an adult,

you can

still decide what you want at any time.

If you

feel that way.

I completely agree with Wierd.

I also think it's important that this be raised, because one of the major

problems we're facing now is that there's an explosion in the

number of people at gender clinics in the Netherlands.

And we don't know exactly where that comes from.

That could be because it’s simply more accepted.

Some people say that. But it could also be that some

people have

started identifying as trans without actually being trans.

And those are, for example, people who struggle with autism.

That’s something that also occurs there fairly regularly.

So these are people Wierd says you treat as if they’re trans

So you do indeed mutilate bodies without that helping them in the long

So you do indeed mutilate bodies without that helping them in the long

term. And that makes this one of the greatest tragedies

if

that really is the case.

The number of girls.

With everything. I just can hardly imagine it.

Among other places, this happens in a hospital in Amsterdam.

the hospital.

I can’t imagine that a doctor

knowingly prescribes medication, prescribes hormone treatment even though

he knows that in the long term, this may not be good.

Because they aren't allowed to have doubts about this kind of thing.

If you go to a doctor and you, you say, “I'm transgender,”

then it doesn't make much sense for that doctor to say, “Let's take a critical look at that,”

because that is seen as denying

the identity of that person.

the identity of that person.

And those doctors don't dare to do that.

There is also a great deal of ideological and political pressure, of course,

so you shouldn't forget that either. And then such a…

Yes, doctors often go along with what is expected of them,

what

is socially acceptable.

We have… There was a, a poll conducted by Maurice de Hond

on this subject.

Some quite remarkable figures are emerging about what people in the Netherlands think.

Because it is being said that what you just mentioned, Victor,

that there is also often a case

of other mental health issues.

So what should happen to those children and young people under eighteen?

And then 75% say psychological treatment, not medical treatment yet.

Only 5% say that. Remarkably, when you look at

the political affiliations

of these people, you also see that among left-wing parties, a

majority

believes the treatment should be psychological, and therefore not medical.

I didn't expect that.

No, but you're making it a bit of an either-or conversation.

—

But I don't think that's the case. So you can go to three places in the Netherlands

for help

if you do want hormone treatment.

Or, indeed, puberty blockers.

So in Nijmegen and Amsterdam, and I think in Zaandam or Zaanstad as well.

I have to assume that.

The Dutch Protocol naturally came about because those children were

depressed, sad, and self-mutilating.

Okay, so how can we help these children

without immediately

performing surgical procedures?

—

That has to happen. The children will certainly, definitely have five conversations with a doctor before that step is taken.

—

You have to be in a very stable family situation.

So doctors also want to be absolutely sure:

you're not all alone in a room, in your little room? Do you have…

Are your parents present? Do you have a stable circle of friends?

Yes.

So the Dutch Protocol may be very different in Finland.

In Sweden.

One small final point, perhaps.

Yes, and here in the Netherlands.

This is taboo on talk shows. This is potentially a major problem and

a

an important debate to have. I think the media shouldn't shy away from it

and therefore

it should be discussed much more often on all programs.

Hoe moeten we daar nou medisch mee omgaan?

Ja,

Of inderdaad puberteitsremmers.

In Zweden.
